Submit your report

Choose the type of report and follow the steps to share your message with the Ombudsman. You can choose to identify yourself or submit your message anonymously.

How to submit your report

  1. Choose the type of report: select compliment, suggestion, request, complaint or misconduct report.
  2. Tell us whether you want to identify yourself: you can submit your report anonymously or choose to provide your details.
  3. Fill in your contact details: if you choose to identify yourself, provide your details so the Ombudsman can contact you and send updates.
  4. Describe the situation: report the facts clearly. For complaints or misconduct reports, also provide the date and time of the occurrence.
  5. Attach files if needed: send documents or files that help us understand the situation. Then review the information and submit your report. Attachments are optional.

Use this channel to submit a compliment, a suggestion, a request, a complaint or a misconduct report. Choose the type of report to “Start”.

In the next steps, you can decide whether you want to identify yourself and share the details of your message.

You can submit your report with or without identification. If you identify yourself, the Ombudsman will be able to contact you and send updates on the progress of your case. If you choose not to identify yourself, you will not be able to receive these updates. The data provided will be treated confidentially, in accordance with the LGPD (Brazilian General Data Protection Law).

For complaints or misconduct reports: provide the date and time of the occurrence.

Formats: PDF, PNG, JPG, MP4, MP3, MOV and WMV. Up to 4 files of up to 128 MB each; the actual limit depends on server limits and permissions.
